Required Documents for Primary One Registration Exercise in Singapore

For SY 2010, taken from MOE website

A: For Children who are Singapore Citizens

1. The child’s Singapore Birth Certificate.
2. The child’s Singapore Citizenship Certificate for those who are not Singapore Citizens at the time of birth.
3. Singapore NRIC of both parents or Entry / Re-entry Permits of parents if they do not possess Singapore NRIC.


B: For Children who are Singapore Permanent Residents

1. The child’s Birth Certificate.
2. The child’s Entry/Re-entry Permit. This information can be found in the parent’s Entry Permit or passport if the child’s photograph appears in the parent’s travel document.
3. Singapore NRIC of both parents or Entry/Re-entry Permits of parents if they do not possess Singapore NRIC.

C: For Children who are Non-Singapore Citizens and Non-Singapore Permanent Residents

1. The child’s Birth Certificate.
2. The child’s Passport and Dependant Pass (if applicable).
3. Parents’ travel document/Singapore NRIC/Entry or Re-entry Permit/Employment Pass/Work Permit.

D: Immunisation Certificates

All parents should produce the immunisation certificates (diphtheria, pertussis, tetanus, poliomyelitis, measles, mumps, rubella, Hepatitis B and BCG) of their children, if available. Please see   immunisation requirements for details.

E: Letter of Authorisation

Parents may issue a letter of authorisation to their relatives or friends to register their children. The letter of authorisation and the relevant documents required for a particular phase are sufficient for registration purposes. For families who are overseas at the point of registration and cannot produce the originals of the relevant documents, photocopies of these documents together with the written evidence of overseas posting can be produced by the proxy authorised by the families. However, the originals of these documents must be produced for verification by the school before 2 Jan 2010.

F: Statutory Declaration

Statutory declarations are required where parents may enroll their child in a school within 2 kilometres from the place of residence of either of the child’s grandparent or parent’s sibling. For the purpose of registration, both the child’s grandparent or parent’s sibling and one of the child’s parents are required to make a statutory declaration at the MOE Customer Service Centre. You need to produce the original copies of the following documents:

* Identity card of both parents
* Identity card of child’s grandparent or parent’s sibling
* Birth certificate of the child to be registered for admission to school
* Documents to prove the relationship between parent and the child’s grandparent or parent’s sibling
* For parents who are self-employed, please bring along supporting documents to prove their employment e.g. Business File or CPF statements or Income Tax
* Photocopies of the passport/NRIC/overseas employment letter is required for parent/s who are currently overseas
* Employment letters to show full-time employment where available

G: For Registration under Phase 2A(1)

In addition to the documents listed in A or B and D, relevant documents certifying the status (eg. member of the School Advisory/Management Committee, or former student of the school and the school’s alumni association) of the parent should also be provided. These documents are to be retained by the school until the child has completed his/her studies in the school.
H: For Registration under Phase 2A(2)

In addition to the documents listed in A or B and D, relevant documents certifying the status (eg. staff or former student of the school) of the parent, brother or sister and also the relationship between the brother or sister and the child should also be provided. These documents are to be retained by the school until the child has completed his/her studies in the school.
I: For Registration under Phase 2B

In addition to the documents listed in A or B and D, a letter from the organisation to certify that the parent is a member endorsed by the organisation directly connected to the school should also be provided. These documents are to be retained by the school until the child has completed his/her studies in the school.
J: For Registration using address of yet-to-be completed property

An original Sales and Purchase document is required if the address of a yet-to-be completed property is used for registration. A copy of this document is to be retained by the school. Parents using the yet-to-be completed property address for registering their child in a school of choice must move into the new property within two years of the child’s entry into Primary One. For this purpose, the date of commitment by the developer in T.O.P (Temporary Occupation Permit) must not be later than 31 Dec 2011.
